    • Responsibilities may include the following but are not limited to:

  • Traveling to patient's home and providing Best Practice nursing care
  • Checking vital signs
  • Documenting in a complete and confidential manner
  • Evaluate the patient’s wellbeing, reporting any needs to the Director of Clinical Nursing or emergency services
  • Providing personal care and meals
  • More advanced nursing such as chest tubes, IVs, colostomy, urinary catheter management are also available
  • Giving emotional support to client and families in times of crisis

    • Requirements:

  • Valid LPN license
  • Annual physical within one year
