Licensed Practical Nurse (LPN)

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our mission-driven healthcare team.
In this role, you'll deliver high-quality nursing care, support treatment plans, and collaborate with physicians and other healthcare professionals to achieve the best outcomes for our patients- many of whom are from underserved communities.
Key Responsibilities

o Provide direct patient care within your scope of practice, following established protocols.
§ Patient triage for same-day urgent and post ER/IP admissions.
§ Patient contact management of post ER and IP admissions
§ Direct Patient education, i.e. medication, post procedures directives, pre-procedures preparations, changes to clinical procedures or physician orders
§ Supplemental facilitation of laboratory requests and clinical encounter processing.
§ Assistance with clinical procedures as directed by licensed practitioners...
