Summary The incumbent functions as a Licensed Professional Mental Health Counselor (LPMHC) in the Mental Health Residential Rehabilitation Treatment Program (MH RRTP) located at the Dayton VA Medical Center. Responsibilities The incumbent functions as a Transitional Care Manager whose primary responsibilities are to provide a full range of direct patient care services, including psychotherapy and discharge planning, to MH RRTP Veterans diagnosed with a range of Substance Use Disorders, Mental Health Disorders, and experiencing psychosocial issues including homelessness. Duties: Provides a range of direct patient care services, to include individual and group psychotherapy, psychoeducation, assessment and diagnosis, psychoeducation, family/couples therapy, and case management/discharge planning.
