The LCSW provides integrated behavioral health services, delivering assessments, brief therapeutic interventions, and care coordination for geriatric patients.This role supports improved psychosocial functioning,facilitatesaccess toappropriate levelsof care, and collaborates closely with providers and the multidisciplinary team to ensure comprehensive,patient centeredcare.
Key Responsibilities

Conduct behavioral health assessments and develop individualized treatmentand transition plans.
