Primary duties and responsibilities
* Comprehensive patient care : Provide care for a panel of patients with multiple co-morbidities, focusing on prevention, diagnosis, and treatment of acute and chronic conditions. Prevention of unnecessary ER visits and Hospitalizations. Identifying high risk members on daily basis and bringing them to discussions during high risk huddles.
* Multidisciplinary collaboration : Work closely with a team that can include advanced practice providers, case managers, specialists, and others to create and execute patient-centered care plans.
