About this role:

As anAdvanced Practice ProviderHome InstaCare,you will work with a team of healthcare professionals committed to providing quality care and excellent patient experience. In thisrole,you willprovide Urgent/ InstaCare services to patients in their home.

• This position includes a$5,000 sign-on bonus, up to$7,500 in relocationassistancewhen applicable, a competitive salary, andan$18,000.00 Annual Quality Incentive

• Your home base will be Clinic Wynn this role isprimarilyamobile practicedelivering care to the patient in their homesettingvia an Intermountain car/vehicle

• Weekly schedule involves 3 x 12 hour days/ week
