Description

Reporting to the Senior Manager, Client Care, the Integrated Care Manager provides leadership within an Indigenous‑led, holistic model of care. The role supports day‑to‑day operations, team development, and coordination of interdisciplinary health and wellness services at SOAHAC’s London site. It works closely with Traditional Healing teams, physicians, nurse practitioners, mental health and addictions workers, allied health professionals, and community partners to support integrated care pathways grounded in cultural safety, relational practice, and Indigenous wellness approaches.
