The Intake Coordinator will be responsible for thetimely coordination of the referral processincluding thereview/response/coordination of carefor patients referred to Klarus Home Health/Jet Health, Inc.


A Licensed Practical Nurse degree is highly preferred but not required.

Requirements


+ Coordinating the referral process for patients referred to Klarus Home Health/Jet Health, Inc.

+ Monitoring, reviewing, and responding to patient referrals.

+ Developing referral relationships within the community.

+ Interacting with facility representatives, discharge planners, and other agencies to facilitate care...
