IHA Nurse Practitioner

The Judge Group is currently seeking Nurse Practitioners (NPs) and/or Physician Assistants (PAs) to conduct in-home health assessments across Tioga County , New York .
Position Summary

• Providers will conduct in-person, no-cost annual preventive health risk assessments for members enrolled in Medicare, Medicaid, and Affordable Care Act (ACA) plans.
• Assessments take place in the member's home and must meet all health plans, quality and service expectations .
• Minimum requirement: 15 visits per month.

• Providers must be credentialed with Excellus, NYS Medicare, and Medicaid to avoid onboarding delays.
• Candidates must be willing to travel within a 50-minute radius .

• Flexible daytime and evening availability
• Appointment duration: 60-75 minutes

• Paid at market rate per completed assessment
