Responsibilities

• Managing a caseload of approximately 10-12
• Home care cases
• MCA work
• Safeguarding
• Hospital discharges
• Community assessments

Working pattern

Initially fully onsite. Once settled, option to work 2 days from home / 3 days onsite.
