Humanitarian Settlement Program Case Manager

Position Summary

The Humanitarian Settlement Program (HSP) Case Manager works collaboratively as part of the HSP Team to provide high level case management, information and referral services to HSP clients during their first 18 months in Australia. This role works within program guidelines to empower individuals to feel safe and secure, to promote their wellbeing, and to enable their full participation in society through connection to culture and community.
The HSP Case Manager is responsible for the development and delivery of the Case Management plan, which will be achieved through developing key collaborative relationships with Cultura Settlement and Community services staff, and on occasion external service providers.
The successful applicant will align their practice with the Settlement Council of Australia (SCOA) Settlement Sector Quality Framework and related Settlement Outcome Standards.
