PROVIDER MUST HAVE ACTIVE WA License or IMLC AND WA DEA for this assignment as they will be treating patients in the southwest WA area as it is considered apart of the Portland metro area.

MUST have the OR License and OR DEA LICENSE MUST BE IN HAND TO BE PRESENTED TO THIS NEED.
