Hospice Social Worker

**Job Description:**
Provides social work services and discharge planning to patients and families in the healthcare setting to meet individual health and psychosocial needs in the promotion of wellness and cost-effective outcomes. This position also transitions patients to alternate levels of care and provides crisis intervention as needed.

**Education Qualifications:**

+ A bachelor degree in social work from an accredited social work program

+ Current State Social Work Licensure

East Range Hospice:

+ Possession of a valid motor vehicle operator’s license as verified through appropriate licensing agency.
