Hospice Social Worker

Serenity Hospice Care is looking for compassionate and dedicated Social Worker to join our team! This position will be responsible for performing the social services section of the assessment process as well as provide on-going care to patients and their families. This position will oversee Monmouth County.

Responsibilities:


+ Managing and completing all psych-social assessments

+ Educating the patient and family about services

+ Managing financial information from patients and family

+ With the interdisciplinary team, develop a plan of care.
