Hospice Registered Nurse Case Manager

Position Overview

The Hospice RN Case Manager provides comprehensive case management and skilled nursing care to hospice patients in their homes. This role focuses on symptom management, coordination of interdisciplinary services, patient and family support, and end of life care aligned with hospice philosophy.

Schedule & Work Details

• Coverage Areas: Clallam and Jefferson Counties, Western Washington


• Schedule: Monday through Friday, 8:00 AM to 4:30 PM


• Weekly Hour Guarantee: 32 hours


• On Call/Weekend Requirement: Occasional coverage when night or weekend staff call off


• Mileage Reimbursement: Reimbursed between patient visits only; commute to and from work is not reimbursed


• Pay Rate: $51 to $53 per hour
