Hospice Nurse Practitioner

Named by The Boston Globe as one of the Top Places to Work in Massachusetts for the last four years, our community-based, mission-driven hospice and palliative care organization seeks an experienced and compassionate nurse practitioner to join our dynamic team in a full-time role. The hospice nurse practitioner will join our expanding interdisciplinary team providing care for hospice patients in the community setting.



Job Responsibilities :


+ Complete face-to-face visits for initial and ongoing hospice eligibility, assessing disease-related changes, declines in functional and cognitive status, and symptom burden, including effectiveness of current interventions

+ Communicate significant visit findings (including unmanaged symptoms and eligibility concerns) to the designated hospice physician, in a timely fashion
