Position Overview
We are seeking an experienced Registered Nurse to provide skilled home health services with a primary focus on OASIS assessments and coordination of care. This role serves patients across Monongalia, Marion, and Harrison Counties and requires strong clinical judgment, accurate documentation, and collaboration with the interdisciplinary care team to ensure high quality patient outcomes.
Schedule & Work Details
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- Weekly Hour Guarantee: 32 hours
- Productivity Expectation: 25 to 30 points per week
- Coverage Area: Monongalia, Marion, and Harrison Counties
- On Call/Weekend Requirement: At least one weekend per month, one Friday, and four additional weekdays as part of the rotation
