Position Summary

The Home & Community Care Nurse provides professional nursing services to clients within the community. The successful candidate will deliver client-centered care, health education, assessments, treatment, and support services to individuals and families in their homes and community settings.

Key Responsibilities

Clinical Care

• Conduct health assessments and develop care plans.
• Provide nursing care and treatment services.
• Monitor client health status and document progress.
• Administer medications and treatments as prescribed.
• Coordinate care with physicians, specialists, and health professionals.
