HIV Community Liason

LaSante Health Center is seeking a dedicated HIV Community Liaison to support individuals living with HIV and enhance community engagement. The ideal candidate will have strong communication skills, knowledge of HIV-related issues, and a passion for advocacy.







Responsibilities:




+ Support individuals living with HIV and connect them to care.

+ Conduct outreach and educational initiatives.

+ Build relationships with community organizations.

+ Advocate for patients and address barriers to care.

+ Provide HIV-related education and resources.

+ Collaborate with healthcare teams to support patients.

+ Maintain records and complete assigned tasks.
