Health Care Assistant

This is an exciting opportunity to join ourevolving Integrated Neighbourhood Team across Bosvena and Three Harbours as aHealthcare Assistant.

Working under the supervision of the LeadNurse, the Healthcare Assistant will support the delivery of safe, proactiveand patient-centred care across the Integrated Neighbourhood Team.

The post holder will undertake a range ofdelegated clinical duties including health checks, phlebotomy, ECG recording,wound care support, learning disability health checks, chaperoning, assistingwith procedures, processing laboratory samples and supporting clinical stock,vaccine and equipment management.

The role will help patients access timelycare and support the wider team to deliver personalised, preventative andcoordinated long term conditions care across our local population.
