Habilitation Specialist 1

Duties Description • Coordinate, develop and provide Habilitation services for Individuals in an IRA setting.
• Develop and modify Community Habilitation and Habilitation Residential Plans; Safeguard Summary Plans; Community Inclusion Plans; Money management assessments and Personal Expenditure Plans as needed.
• Coordinate with Clinical staff to ensure all individual needs are met and programs are developed/modified as appropriate.
• Provide training and hands on assistance in the proper delivery of daily services.
• Provide oversight of inclusion experiences by participating in community activities for individuals.
• Provide training and oversight in the proper documentation of Medicaid billing documents; Community Habilitation and Residential Habilitation Monthly notes; and EHR notes in relation to the implementation of services.
• Develop and maintain a good working relationship with Care Coordinators and staff that serve individuals.
