-Dates: ongoing
- Required rostered hours - minimum 40 hours per week with roster to be agreed with the practice manager
- Services Fee (hourly rate) / % : $187.50 + GST per hour or 70% + GST of billings, whichever is greater in the billing period
- Accommodation: if unable to use SHP lodgings then $150 per night allowance, booked by agency
- Car hire per day booked by agency
- Flights: lowest fare applicable allowed, ideally local QLD or East Coast based GP
