The clinic is looking for a dedicated physician to work either part-time or full-time, taking over a panel of approximately 1,200 patients as the current physician retires or sharing responsibilities for two to three days a week. Strong collaboration with in-house RN and visiting pharmacists allows for holistic patient care across all ages and complexity levels using the Healthlink Health Connect Registry.
Key Responsibilities: • Care for a general patient population of various ages • Collaborate with in-practice RN and visiting pharmacist • Manage and transition a patient panel smoothly • Utilize Healthlink for effective patient management • Choose to practice full-time or part-time
