Veteran Pathways & Proactive Care is seeking experienced General Practitioners for a fully remote telehealth position, delivering structured case conferencing and chronic disease management for veterans and patients with complex, multi-system health needs.

This is not a standard telehealth locum. You will provide clinical leadership within integrated Chronic Care Teams, working collaboratively with allied health professionals and specialists to translate clinical findings into coordinated, evidence-based care plans. The work is structured, purposeful, and genuinely multidisciplinary.
