About the Role:
We are seeking a dedicated and compassionate General Practitioner to join our respected Aboriginal health service in Swan Hill. As a key member of our multidisciplinary team, you will collaborate with nurses, Aboriginal Health Workers, and community workers to provide culturally appropriate healthcare to Aboriginal and Torres Strait Islander individuals.
In this role, you will focus on ensuring a welcoming environment and delivering high-quality care tailored to the unique needs of our community. Your responsibilities will include providing comprehensive case management for patients with complex and chronic conditions, conducting mental health consultations, and facilitating referrals as needed.
Additionally, you will provide GP services within the clinic and offer outreach services to the nearby town of Kerang one day per week. There is also the opportunity to provide VMO services at Swan Hill District Hospital.
