**Full time Social Worker - Hospice Columbia and Greene Counties**

The Community Hospice has a full time opportunity for a social worker to join our hospice care team caring for patients in primarily the Columbia county area as part of our Interdisciplinary Team in homes and Nursing homes.

As a member of the hospice interdisciplinary team, the primary focus is to respond to the emotional and psychological needs of patients, families and staff.

**Position is Full time Monday- Friday 8a-4:30p.**
