Flow Coordinator

Are you looking for an exciting change of role in an established but developing service? We are looking to recruit two dynamic and innovative Band 5 Flow Coordinators to join the Integrated Community Response Service – a multidisciplinary team made up of GPs, an Advanced Clinical Practitioner, occupational therapists, a social worker, physiotherapists, nurses, a community matron, and support workers. The main aim of the service is to manage complex clinical cases at home, prevent unnecessary hospital admission and support early discharge of patients following an acute admission.

The team provides immediate assessment, intervention and management to a broad range of patients with complex needs. All members of the team work very closely together and have a good relationship with other community teams and clinicians such as district nurses, GPs, London Ambulance Service and mental health services.
