Overview

An exciting opportunity has arisen for a Family Practitioner to work in Family Help in West Norfolk & Breckland. Your office base will be at Elizabeth House, Dereham.

We are looking for an experienced practitioner who can manage a complex caseload of Family Support cases and work with children and families to enable sustained positive change.

Responsibilities

• Manage a complex caseload of Family Support cases, providing high‑quality support to children, young people and families.
• Work collaboratively with the child, their network and other professionals to identify concerns, produce plans and deliver services to achieve positive outcomes.
• Identify and assess risk, and keep risk under review throughout each case.
