Position Overview:

• Schedule: Full-time, Monday Friday
• Patient Load: patients per day per physician, allowing time for quality, patient-centered care
• Team Model: Work as part of a Patient-Aligned Care Team (PACT )
• Each physician is supported by a dedicated RN , LPN and MSA and has expanded support from clinical pharmacy , social work ,
