What is a Family Model Provider?

A Family Model Provider opens their home and their heart by proving care to individuals with disabilities in a family home setting, as opposed to individuals living in an institutional setting. Family Model Providers provide housing, supervision, support and personal care to one or more individuals. Support may include assistance with daily living activities, such as bathing, personal hygiene, toileting, ambulation, dressing, eating, and meal preparation. Other supports include transporting and attending medical appointments, community outings and recreation, as well as medication administration which requires medication certification by the Family Model Provider, if the person cannot self-administer. The level of care required by the Family Model Provider varies according to the disability and needs of the individual.
