**Job Summary and Qualifications**

**As an ER Discharge Coordinator, you will be responsible for Point of Care (PoC) Patient Bedside scheduling as part of the company’s centralized ER and Urgent Care Follow-up programs. The truest intent of the ER discharge coordinator is a bedside-initiated patient experience, with the objectives of increased contact rate, scheduling immediacy, and improved overall patient experience. You will create and maintain a positive customer experience in a patient-centered, quality-driven environment. **
