Long Contract available to see you past Christmas and New Year - with 6 months on offer (subject to renewal)!
About the Role:
- Conduct comprehensive assessments for individuals across all age ranges and be comfortable with a case-load of S47s.
- Work collaboratively with professionals from health, education, housing, and other key sectors to ensure integrated, person-centred care.
- Develop and implement care plans that are tailored to meet the unique needs of each individual or family. Offer early intervention, crisis support, and long-term planning for vulnerable individuals.
