This role is critical to ensuring smooth admissions, strong census management, and resident-centered social services from admission through discharge.
Key Responsibilities
+ Oversee and direct all social services and admissions operations within the facility.
+ Coordinate and manage the admissions process, including referrals, bed management, and pre-admission screening.g
+ Serve as a liaison with hospitals, referral sources, families, residents, and interdisciplinary team members.
