In this role, you will assess nutritional needs, design effective care plans, and educate families on dietary practices. You’ll play a vital part in team meetings and the continuous quality improvement process.
Key Responsibilities:
• Monitor nutrition and hydration needs amongst residents
• Complete RAI assessments and dietary referrals promptly
• Create and adjust personalized nutrition plans
• Provide training and support to dietary teams
• Collaborate to enhance the dining experience for residents
Requirements:
• Registered with the College of Dietitians of Ontario
