Diabetes Educator - Registered Nurse

Responsibilities

• Educate and advise patients on self‑management skills in both individual and group settings focusing primarily on diabetes and secondarily on other health conditions such as general health & metabolic conditions (weight management & obesity)
• Develop a care plan for patients and collaborate with the multidisciplinary team, as needed.
