Job Description

Coverage Needed
Start Date: 08/03/2026
End Date: 26 weeks from start
Schedule: Monday to Friday, 8:00 AM - 4:30 PM
Coverage Type: Scheduled; No Call
Clinical Details
Patient Load: 25-35 patients per day
EMR: Epic
Support Staff: 1 support staff (LPN, RN, MA) for every 12 patients
Facility Type: Out-Patient Clinic
Requirements
Board Certified 
New York State License in hand
3 years of recent experience
Recent Epic experience
BLS Certification required
Ensure work history is up to date with no gaps of 30+ days
If malpractice is involved, a physician statement is required
