Job Summary

Together4Healthcare PCN, a progressive and forward-thinking network based in Sandwell, is seeking a dedicated Clinical Pharmacist to join our friendly and supportive team. Our PCN currently employs eight Clinical Pharmacists and three Pharmacy Technicians. We aim for an ideal start date in August 2024 but are flexible for the right candidate.

Key Responsibilities

• Clinical Medicines Review: Provide expertise in clinical medicines review, particularly for high-risk groups like the frail elderly and those with multiple co-morbidities.
• Medication Management: Oversee the management of medicines for patients discharged from hospitals, ensuring continuity and safety in medication changes.
• Educational Support: Improve prescribing practices by providing educational support for all prescribers within the practice.
