Specific requirements/restrictions: must be proficient: Yes, Caselogs Required: Yes - proof of epidurals, spinals and blocks, Vaccination Required: Following the NM state mandate, everyone is to be vaccinated or follow the exemption requirements, NM Controlled Substance license required.
