Complex Needs Coordinator

THE ROLE
The Complex Needs Coordinator provides consultation and coordination services for clients with complex support needs and is a single point of contact for service providers, clinicians or people seeking general information about complex needs service responses/interventions. Complex needs service responses/interventions include Multiple and Complex Needs Initiative (MACNI), the Support for High Risk Tenancies (SfHRT) program and broader complex needs responses.
The position will:
* Provide consultation and coordination services and facilitate service planning for clients with complex support needs, including MACNI and SfHRT clients
* facilitate relationships between departmental program areas and external services, as well as provide capacity building regarding support provision for clients with complex needs
