Job Overview

Band 6 Community Frailty Nurse – an exciting opportunity to join the Rehabilitation and Frailty Team. The role involves providing high standard, patient‑centred care to community residents predominantly with physical conditions, working closely with community nurses, matrons, care coordinators, physiotherapists, occupational therapists and rehabilitation assistants.

Main Duties

• Deliver treatment, advice and teaching within your professional competence, demonstrating evidence‑based care.
• Work 08:00–20:00, 365 days a year, across Basildon, Wickford, Billericay, Brentwood and Thurrock areas.
• Provide individualised care to patients under the supervision of the Clinical Lead Frailty Nurses.
• Respond flexibly to service needs, including weekends and bank holidays.
