Community Nurse

As a Band 5 Community Nurse, you will deliver high-quality, patient-centred nursing care to individuals in their own homes or community settings across the South Hams area. You will work as part of a multidisciplinary team (MDT) supporting people to maintain independence, manage long-term conditions, and avoid hospital admissions wherever possible. This position is pivotal in the shift from hospital based care to preventative neighbourhood care and response.

Main duties of the job
