Community Mental Health Nurse

Overview

Community Mental Health Nurse role for the Lancashire & South Cumbria NHS Foundation Trust, working 0.8 WTE (30 hours) as a Band 6 Mental Health Nurse Practitioner with the Lancaster and Morecambe Older Adults Community Mental Health Team (CMHT). The closing date is 18 June 2026.

Main duties of the job

• Experience of working with older adults with mental health needs and the ability to lead on assessment and provide treatment plans.
• Excellent communication skills.
• Work within a Multi Disciplinary Team to support older adult service users on the dementia diagnostic pathway and in the community to remain at home and prevent hospital admission.
