Job Overview

This job opportunity seeks passionate and community-oriented individuals to join the Virtual Ward Team as Band 7 Community Matron. The position emphasises admission avoidance through rapid response assessments and hospital‑at‑home interventions, providing patients with alternatives to hospital admission. The service runs from 8am to 8pm, covering those hours.

Key Responsibilities

• Collaborate with the UCR, Integrated Front Door, and Home‑Based Intermediate Care multidisciplinary teams (MDTs) to deliver effective interventions.
• Oversee patient care, ensuring a holistic approach to meeting the needs of patients within their homes.
• Support from a broad team, including Consultants, GPs, Advanced Clinical Practitioners (ACPs), and the Integrated Care Service (iCares), which spans across Sandwell Community.
