Community Health Representative

**Full-Time| M-F, 8am-4:30pm (ON CALL 24hrs Mon-Sun) | PG 6| Weekly earned wage access is an option.**
**Job Summary or Objective:** The Community Health Representative provides in-home care, health education, and support services to tribal members. This role coordinates appointments, helps with medications, and ensures culturally appropriate communication and outreach.
**Primary Tasks:**
1. Serve as a liaison between Indian communities, tribal programs, and facilities through regular home visits to ensure patient medical needs are met.
2. Conduct individualized home assessments, monitor vital signs, provide disease prevention education, and remain on 24-hour call due to the nature of the role.
3. Assist with scheduling appointments, arranging transportation, refilling medications, organizing weekly pill planners, and translating instructions to match patients’ cognitive and language needs.
