Community Coordinator / Filed worker

Job Title: Community Coordinator Reporting to: Program Coordinator Job Purpose: To facilitate the community-based rehabilitation (CBR) of persons with spinal cord injury (PwSCI) by coordinating identification, goal-setting, referrals, service delivery, stakeholder engagement, and documentation. Key Responsibilities: 1. Conduct functional assessments and provide therapy to PwSCI through home visits, camps, and tele-rehabilitation. Refer cases to specialized rehabilitation centers and coordinate follow-up care. 2. Work closely with PHCs, hospitals, and local service providers to strengthen referral systems and ensure availability of self-care kits, medical support, and assistive devices. 3. Organize training sessions for parents, caregivers, and local stakeholders. Lead sensitization programs and support the formation of peer support groups or district-level federations. 4.
