As a Community Care Professional (Evenings & Weekends) you will provide essential care and support to clients in the community —enabling clients to maintain dignity, independence, and quality of life within their own homes;
Key Responsibilities
Deliver high-quality, person-centred care in clients’ homes
Support with daily living tasks such as washing, dressing, mobility, light household duties and meal preparation
Provide companionship and build positive, trusting relationships
Assist with medication prompts and general wellbeing monitoring
Accompanying your client to appointments, or a visit to the shops
Record and report care activities accurately
Promote independence, respect, and choice in every interaction
Communicate effectively with clients, families, and team members to ensure seamless care coordination
