Community Based Treatment Provider II

Job Summary

The Community Based Treatment Provider II provides in home and community-based therapeutic interventions to youth and families to assist in behavioral and mental health symptom management. This position provides Community Psychiatric Supports and Treatment (CPST) within Child and Family Treatment and Support Services. May provide Psychosocial Rehabilitation (PSR) and Planned Respite (PR) within Child and Family Treatment and Support Services as well as Home and Community-Based Services programs, depending upon the region.

Essential Job functions

+ Assess youth and family needs and create individual service plans to meet those unique needs.

+ Update the individual service plan regularly in collaboration with the child, youth, and family and multi-disciplinary treatment team.
