Provide high‑quality personal care and holistic support to patients and their families in the comfort of their own homes, helping them to remain at home at end of life in line with their preferences.

Responsibilities

• Deliver personal care (dressing, mobility, mouth‑care, repositioning, skin‑care, toileting, washing, etc.).
• Provide emotional support, last‑wishes guidance, skill‑sharing for family members, and transition support from hospice/hospital to home.
• Assist with temporary bridging of personal care while statutory arrangements are sought.
• Travel across the five London Boroughs of Bromley, Croydon, Lambeth, Lewisham and Southwark to support patients.
• Attend the Sydenham office each day to review the schedule and plan care.
• Participate in rotational work on the inpatient unit for professional development and continuous practical updates.
