Reporting to the Manager, Pharmacy Clinical Programs, the Clinical Pharmacist, Community Health Services works collaboratively with the Seniors, End of Life and Home & Community Care teams, is responsible for practicing client centered care by identifying and resolving potential drug therapy problems in the community setting; provides medication counseling and drug information to clients and health care providers; conducts clinic sessions and/or home visits as required; and provides academic detailing to optimize prescribing practices.
