Clinical Pharmacist

Working in general practice alongside a team of pharmacists, they will be required to act within professional boundaries and provide support in medicine optimisation and to conduct clinical medication reviews.

Main duties of the job

• To conduct clinical medication reviews and proactively manage complex polypharmacy patients, and reduce inappropriate prescribing where possible.
• To take responsibility for the management of chronic diseases within the practice
• To support the repeat prescription system, and to manage the repeat prescribing reauthorisation process
• To complete medicines reconciliation on transfer of care which includes hospital discharge summaries, and to liaise with the patient and community pharmacists to ensure a continuous medication supply
